A POLICE RECORD SEARCH may be required when accepting
various security positions, driving positions or positions dealing with money or
liquor. As a Volunteer this search will be of no cost to you but may be
necessary to ensure the safety and legal guidelines set out for the Festival and
its staff and Volunteers. IT IS
YOUR RESPONSIBILITY TO ATTEND AT THE POLICE STATION TO SUBMIT TO THE RECORD
SEARCH AND TO PROVIDE PROOF TO THE FESTIVAL OFFICE.
I, the undersigned, personally and on behalf of my heirs, executors, administrators and assigns, hereby release and forever discharge the following:
a)
The Collingwood Elvis Festival Inc., the Town of Collingwood and the
County of Simcoe;
b)
All sponsors, organizers and volunteers of the Collingwood Elvis Festival
Inc., their respective officers, directors, agents, representatives or
successors, from any and all claims, responsibility, liability or demands that I
have or may have or my heirs, executors, administrators, assigns or any third
party have or may have for personal injuries and or property damage of any
nature or kind whatsoever, arising out of my participation, in any capacity or
at any level, with the Collingwood Elvis Festival and its program or events.
I
authorize the Collingwood Elvis Festival Inc. to have such care and power to
check and verify all information provided by me, within this application, is
complete, accurate and true to the best of my knowledge and belief, and to act
on my behalf, as may be required by me by medically qualified personnel during
my participation in any event of the Collingwood Elvis Festival, without fear of
consequence or liability whatsoever arising from their actions.
I
have read the above statement and I understand the terms and conditions.
I further understand and accept that my signature upon this document
confirms my acceptance of such terms and conditions.
I attest and verify that I have full knowledge of the risks involved in
my participation and that I am physically fit and able to participate in said
festival events.
