The
Tribute Artist hereby understands and agrees to participate in all facets of the
program. The Tribute Artist further agrees that his/her likeness and voice may
be used in any promotional and/or audio/video reproduction as a result of their
participation in the Collingwood Elvis Festival 2002, and that no remuneration
will be paid for such performances or appearances. All Tribute Artists, whether
Competitive or Non-Competitive, hereby understand and accept that they are
contractually responsible, upon signing of this registration form, to be ready,
willing and available to participate in the Festival events as outlined below.

REGISTRATION FEE: $50.00 CDN for all categories, Competitive, Non-Competitive or Youth.
*Please note that your registration fee will not be cashed or processed until the selection process has been completed. You will receive notification by phone regarding the outcome of the selection process.
2. CD/CASSETTE OR VIDEOTAPE - Minimum 2 songs - one fast tempo & one slow tempo/ballad. ELVIS SONGS ONLY. For selection Committee use only and will not be returned!
3. PHOTO IN FULL COSTUME. Colour of Black & White is acceptable. This photo will also appear in our souvenir program.
4. A CONDENSED RESUME - Highlights of performances, appearances, contest etc. which you have been involved in. Please include karaoke, festivals, private functions, venues, etc.
5.
A short 5-line
bio about yourself. Please note your bio may need to be altered by our editorial
staff
pending space requirements. This Bio will appear with your photo in our
Souvenir Program
