You authorize the Collingwood ELvis® Festival Inc. to have such care and power to check and verify all information provided by you, within this application, is complete, accurate and true to the best of your knowledge and belief, and to act on your behalf, as may be required by you by medically qualified personnel during your participation in any event of the Collingwood Elvis® festival, without fear of consequences or liability whatsoever arising from their actions.
